Compensation (Salary): $99,822.57 (Annually)

Location: Old Town Recovery Center (33 NW Broadway, Portland, 97209)

Schedule: *8:30am - 5:00pm (Monday - Friday)

*Note: This schedule is subject to change. Please note the RN Case Manager role may require you, at times, to be available after hours, and during the holidays

 

MINIMUM QUALIFICATIONS (REQUIREMENTS)

*Note: Prior to submitting your application, please ensure your professional/working experience reflects the requirements listed below:

  1. Required: Successful completion of a course of accredited study that leads to the AD, or BSN or AA degree in Nursing from OSBN.
  2. Required: Current RN license in the State of Oregon.
  3. Required: Current certification with MHACBO as a QMHA-R or QMHA I.
  4. Required: A minimum of two (2) years experience in psychiatric or addictions nursing.
  5. Required:  Must possess a current driver’s license.
  6. Required: Must be available for after hours pager and holiday coverage rotation.

 

PREFERRED EXPERIENCE (NOT REQUIRED)

  1. QMHP.

 

ROLE SUMMARY:  The RN/Case Manager works as a member of a multi-disciplinary team providing intensive case management and integrated dual diagnosis treatment. The team works to address all treatment needs, including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Participate in Outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort clients to meetings and appointments as necessary.
  2. Collect information regarding clients’ history through a combination of interviewing, records review, and consultation, after obtaining written consent.
  3. Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation.
  4. Develop treatment plan with client.
  5. Provide direct care and delegate care of patients as indicated by patient needs.
  6. Triage and assess clients in clinic & community settings.
  7. Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients
  8. Formulate individualized nursing care plans and participate in patient care meetings.
  9. Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
  10. Administer medications and treatments as directed by the standing orders.
  11. Consult with other health team members to lend guidance as necessary.
  12. Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning.
  13. Participate in admission and discharge planning for any client needing hospitalization or subacute level of care. Client contact must be made within one working day of admission.
  14. Monitor and assure that the health and safety needs of clients are met.
  15. Coordinate services with all members of client’s support network.
  16. Maintain client records as required by program standards and funding requirements including documentation of clinical/medical interactions, behavioral health assessments & individual service plans.
  17. Participate in the rotation of carrying after-hours pager and providing holiday coverage with other members of team/program.
  18. Attend all mandatory CCC meetings and trainings in a timely manner
  19. Monitor patient condition, note changes in status, utilize judgment and take appropriate action.
  20. Assist the staff physician.
  21. Accurately and concisely document patient care activities and information.
  22. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.  Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.  This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

 

DESCRIPTION OF OTHER DUTIES:

  1. Conduct educational and therapeutic groups for clients, using approved curriculum.
  2. Provide quality care in an efficient, cost effective manner.
  3. Make referrals to hospitals and other agencies.
  4. Participate in compliance, quality assurance and quality improvement activities as directed.
  5. Perform other duties as assigned.

  7. Knowledge of nursing theory and practice.
  8. Advanced knowledge and skills in counseling and assessment techniques with individuals and groups including assessing for risk of danger to self & others.
  9. Exhibit good nursing judgment in the management care of the emotionally ill.
  10. Application of knowledge in nutrition, sanitation, and personal hygiene.
  11. Ability to follow oral and written directions and to apply techniques for specific needs.
  12. Knowledge of community resources.
  13. Ability to do venipuncture.
  14. Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
  15. Physical ability to move and restrain clients in an emergency situation.
  16. Sufficient manual dexterity and physical ability to perform assigned tasks.
  17. Ability to successfully interact with a wide variety of people including the clients, the manager and co-workers.
  18. Ability to work in an environment where people may be hostile or abusive.
  19. Ability to manage time and meet deadlines.
  20. Excellent communication skills including diplomacy, tact, effective “bedside manner” with patients.
  21. Ability to work in a multi-disciplinary, team-oriented environment.
  22. Excellent problem-solving and conflict resolution skills.
  23. Ability to keep accurate and appropriate records/documentation.
  24. Ability to display non-judgmental attitude.
  25. Ability to learn new skills.
  26. Knowledge and skills in mental health treatment methods and crisis intervention.
  27. Knowledge and skills in chemical dependency treatment methods and crisis intervention.
  28. Ability to communicate clearly and concisely both orally and in writing.
  29. Ability to work courteously and effectively communicate with the general public, clients, medical personnel, corrections personnel, police, and co-workers.
  30. Ability to maintain accurate records and necessary paperwork.
  31. Ability to provide leadership.
