The Social Worker completes comprehensive, culturally competent assessments including consideration of the relevant biomedical, psychosocial, and spiritual factors of the patient and family's needs. When you join our team, you will be part of a supportive caregiver family that will be united in shared values and purpose to fulfill our promise of being the best place to receive care and the best place to work in healthcare. 

 

A caregiver in this position works 8:00am—4:30pm. 

 

A caregiver who excels in this role will:  

  • Interact with patients, families, physicians, and health care team colleagues using patience and tact.  

  • Demonstrate commitment to the process and the Behavioral Expectations in all interactions and in performing all job duties. 

  • Respond to campus wide referrals same working day. 

  • Implement appropriate social service interventions and continuing care options through the process of data collection, data analysis and implementation of a care plan based on identified needs. 

  • Utilize a multidisciplinary approach to ensure continuity of care and comprehensive patient services to maximize the effectiveness of social service interventions. 

  • Develop and sustain positive working relationships with patients, families, physicians, nurses, and ancillary personnel to promote open communications. 

  • Act as a resource to patients, families, physicians, and staff regarding internal/external resources. 

  • Initiate team conferences to facilitate discussion/consensus regarding complex cases. 

  • Use crisis intervention techniques to calm, comfort and stabilize patients/families under stress. 

  • Inform the patients, families, and anyone accountable for the continuing care of the patient of their role and responsibilities relating to the continuing care plan. 

  • Document on CM progress notes, multidisciplinary problem list and interdisciplinary education records to provide clear, concise records of social work services and interventions. 

 

Minimum qualifications for the ideal future caregiver include:  

  • Master's degree in Social Work 

  • One year of clinical experience 

  • Strong organizational and prioritization skills and demonstrated leadership skills 

  • Excellent communication skills adaptable to individual levels of understanding 

  • Critical thinking, problem solving and deductive reasoning skills.  

  • Can participate collaboratively with all members of the care team. 

  • Basic computer skills with familiarity with Windows based software programs. 

  • Crisis intervention, psychosocial and therapeutic counseling experience. 

  • Utilization management, managed care, quality, and payor issues experience. 

 

Preferred qualifications for the ideal future caregiver include:  

  • State Licensed Clinical Social Worker (LCSW) 

  • Two years of experience in hospital/healthcare Social Work 

  • Knowledge of acute care regulatory/accreditation requirements

Physical Requirements:

  • Requires frequent walking from department to department.

  • Requires sitting at a work-station or desk.

  • Requires standing.

  • Work may include occasional publishing and/or pulling, lifting and carrying objects weighing up to 20 pounds, such as files, documents and computer printouts.

  • Work requires finger dexterity and eye/hand coordination to operate a computer keyboard at a moderate skill level.

  • Follows Standard Precautions using personal protective equipment as required for procedures.
