CCHP is looking for a Director of Grants and Special Projects to be part of our exciting team!

JOB SUMMARY

CCHP is seeking a driven, strategic, and results-oriented leader to serve as the Director of Grants and Special Projects, overseeing grant funding, strategic initiatives, and performance measurement to strengthen service delivery. This role will identify and evaluate funding opportunities, manage grant implementation and compliance, and collaborate with internal teams and external partners to drive impactful projects. Under the direction of the COO, special projects include assessing community needs – such as women’s health and services for formerly incarcerated or justice-involved individuals – and developing strategic plans to enhance programming. Additional initiatives may involve piloting a patient engagement app, and leading CCHP’s involvement in the Medicaid 1115 waiver. The ideal candidate thrives in a fast-paced, mission-driven environment, excels in grant oversight, project management, and data-driven strategy, and is passionate about securing resources and leading initiatives that create meaningful change in community health.

KEY RESPONSIBILITIES :

Grant Development and Management

  • Identify grant opportunities that align with CCHP’s goals, collaborating with grant writer consultants and the Senior Leadership as appropriate.
  • Provide comprehensive summaries to the Executive Team of grant opportunities, including financial implications and overall strategic benefits to the organization.
  • Manage grant writer consultants to develop competitive grant applications.
  • Oversee the implementation of funded grants, including monitoring project progress, budget management, and compliance with reporting requirements.
  • Provide technical assistance to program staff managing grant-funded projects.
  • Ensure timely and accurate grant reports to funders, highlighting project achievements and impact.
  • Draft and update policies related to operational changes for new grant-funded programs.
  • Prepare and deliver presentations for grant related meetings with stakeholders.
  • Maintain robust internal grant management systems to track proposal submissions, funding awards, and reporting deadlines.
  • Manage the re-application processes for recurring grants.

Strategic Leadership Initiatives

  • Lead the execution of special initiatives, as designated by COO.
  • Oversee the planning and management of a variety of strategic projects as designated by the COO, including but not limited to financial opportunities, community need assessments, partnerships, technology assessments/implementations, and facility improvements.
  • Collaborate with internal teams to track progress, address challenges, and drive the completion of projects.
  • Be responsible for the direction, coordination, implementation, execution, and control of specific projects.
  • Lead project team meetings, work with vendors, and deliver regular updates to executive staff.
  • Coordinator with staff responsible for monitoring, managing, and completing mid-to-large-scale projects.
  • Be an accountable point of contact for project updates, presenting progress and results to the executive team and board.
  • Anticipate and resolve potential risks and obstacles that could hinder project success.
  • Regularly assess project progress and adjust as necessary to meet shifting priorities or unexpected challenges.

Performance Measurement and Analytics

  • Manage the Business Data Analyst in monitoring and evaluating program outcomes.
  • Collaborate with program teams to develop performance indicators that inform decision-making and improve service delivery.
  • Oversee the creation of performance measurement tools, monitoring reports, and analytics to evaluate program and project performance.
  • Direct the preparation of data and analysis reports that inform organizational policies and procedures.
  • Advise executive staff on data insights, performance findings, and actionable strategies to enhance outcomes.

QUALIFICATIONS

  • Bachelors required (Masters in Business Administration or nonprofit management preferred).
  • Relevant experience in grant research, analysis, writing, management, and compliance.
  • Minimum 5 years’ experience in obtaining and/or managing grants from government, corporations and private foundations.
  • Demonstrated experience in program implementation, including the development, execution, and management of grant funded programs and special projects that address community health needs and improve service delivery.

  • Strong project management skills with the ability to handle multiple priorities.
  • Proficiency in data tracking, budget management, and reporting.
  • Understanding of budgeting principles and grant compliance requirements.
  • Strategic thinking and decision-making.
  • Detail-oriented with exceptional organizational skills.
  • Strong interpersonal and leadership abilities.
  • Knowledge of nonprofit funding landscapes and best practices.
  • Expertise in developing performance metrics and analyzing program outcomes.
  • Proficiency in project management methodologies and tools.

Job Type: Full-time

Salary: $100,000.00-120,000.00 annual
