FULLTIME, non-exempt : 40 hours
Coordinated Care Services Inc (CCSI)seeks talentedFamily / School Support Specialiststo contribute their expertise in providing essential business services to our provider organizations and customers. TheFamily / School Support Specialistsprovide short-term support to youth and their families who face significant mental health challenges.Family / School Support Specialistsassess the needs of the clients they serve, advocating for services and assisting youth and their families in accessing community and school-based services so that the student can find success. CCSI is partnering with Onondaga County Children and Family ServicesFamily Support for Student Success Program (FSSS) (https : / / www.ccsi.org / family-support-for-student-success / ) to hireFamily / School Support Specialists.This position is employed by CCSI and works on behalf of Onondaga County Children and Family Services.
In this role, you will be
- Working jointly with collaborative partners to support family-led goals and coordination of school-based services and support for youth and their families
- Facilitating planned intervention and collaborative problem-solving with students, families, school personnel, and community partners
- Providing goal-driven service planning, school-based assessment, planned intervention, supportive counseling, case management, and ongoing documentation
- Empowering clients by using clients strengths and informal support networks in service delivery
- Other duties as assigned
