Position Summary
The Lead Case Manager I provides ongoing support and expertise to Case Manager I’s through creating initial contact and rapport with emergency shelter clients. The Lead Case Manager I will support and assist Case Manager I’s with facilitating shelter enrollment and diversion services as well as coordinate and participate in the development and delivery of comprehensive Housing First Individual Service Plans. The Lead Case Manager I will assist and guide Case Manager I’s by facilitating housing assessments and coordinating programs and services in accordance with the needs of the clients. The Lead Case Manager I will utilize concepts of Housing First, Harm Reduction modalities, Low Barrier, and Equal Access shelter policies to guide Case Manager I’s in order to provide supportive services to clients and successfully play a crucial role in the client moving from homelessness to home. Working hand in hand with the Assistant Director of Residential Services, Supervisors and/or Managers to guide and coordinate Case Manager I’s to provide clients with “under the hood” services re: medical, mental, dental, transportation etc.
Essential Responsibilities
- Lead Emergency Shelter Case Management team in absence of Supervisor when needed.
- Coordinate breaks and schedules of staff in Emergency Shelter Case Management team.
- Assist with collection of time and activity logs for payroll.
- Maintain awareness of potential resources in the community and make proper referrals.
- Complete a range of required program related documentation utilizing a range of computer programs and data collection systems.
- Utilize electronic records to document and track all case management services and referrals.
- Complete necessary documentation to facilitate referrals to collaborative outside agencies.
- Assist clients with decreasing systems barriers and navigating the social services network.
- Assist with spot checking clients in HMIS/Bridges system.
- Provide HMIS/Bridges system training.
- Provide ongoing support to Case Manager I’s.
- Assist Supervisor by providing guidance to Case Manager I’s in complex client matters.
- Respond to radio request in absence of Supervisor when needed.
- Protect all confidential information, company property and electronic data. Comply with safety rules.
- Other related duties as directed.
Knowledge, Skills and Abilities
- Must be computer literate and possess effective written and verbal communication skills.
- Ability to work independently or with a group.
- Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
- Must have a respect for confidentiality involving both clients and employees.
- Ability to work in a variety of settings with culturally diverse groups. Ability to be culturally sensitive and appropriate.
- Must be able to manage personal appointments and schedule, prioritize work and meet necessary deadlines.
Qualifications
- Two years of full time professional level casework experience in a social service agency preferred.
- Prior experience with homeless population required.
- Experience with chronically homeless men preferred.
- Flexible schedule to work after hours and weekends. Regular schedule may include either Saturday or Sunday each week.
Physical Requirements
- Primarily works in shelter dorm, office environment, but may also work throughout the agency and off site.
- Ability to regularly communicate with clients, co-workers and supervisors.
- Ability to operate and use office equipment, especially computer, keyboard and mouse.
- Ability to continuously sit and/or stand. Ability to walk continuously if needed and climb stairs as needed.
- May occasionally lift up to 25 pounds.
This position pays $19.00 per hour.
