Job Overview
We are seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will work collaboratively with other professionals to develop comprehensive care plans that address the unique needs of each client.
Responsibilities
- Conduct assessments to identify clients' needs and develop individualized care plans.
- Provide crisis intervention and management support to clients in distress.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Supervise and mentor staff members in best practices for client care.
- Collaborate with community resources and agencies to enhance service delivery.
- Facilitate workforce development initiatives to improve team effectiveness.
- Offer addiction counseling and support for clients struggling with substance use issues.
- Maintain accurate documentation of client interactions and progress in compliance with regulatory standards.
- Participate in ongoing training and professional development opportunities.
Qualifications
- Reluable Transportation.
- Experience in behavioral health, child welfare, or social work is highly desirable.
- Strong skills in crisis intervention and management.
- Proficient in motivational interviewing techniques.
- Demonstrated ability to supervise staff effectively.
- Knowledge of workforce development strategies is an asset.
- Experience in addiction counseling is a plus.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team.
- Must live in Merced, or Santa Cruz County
