Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals facing various challenges, including mental health issues, addiction, and housing instability. The Case Manager will work collaboratively with clients to develop personalized plans that promote their well-being and empower them to achieve their goals. This position requires strong interpersonal skills, crisis management abilities, and a commitment to fostering positive outcomes for clients.
Responsibilities
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.
- Provide crisis intervention and management to clients in urgent situations.
- Facilitate access to resources such as Section 8 housing, healthcare services, and workforce development programs.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Mentor clients by offering guidance and support throughout their journey.
- Supervise and coordinate services provided by other professionals involved in the client's care.
- Maintain accurate documentation of client interactions, progress, and outcomes.
- Collaborate with community organizations and agencies to enhance service delivery.
- Offer addiction counseling as needed, supporting clients in their recovery process.
- Provide childcare resources or referrals for clients with children.
Requirements
- Bachelor's degree in social work, psychology, or a related field is preferred.
- Experience in crisis intervention and crisis management is essential.
- Familiarity with Section 8 housing programs is a plus.
- Strong mentoring skills with the ability to motivate and inspire clients.
- Knowledge of Medicare processes is beneficial but not mandatory.
- Experience in workforce development initiatives is an advantage.
- Background in childcare services or resources is desirable.
- Certification or training in addiction counseling is a plus.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a multidisciplinary team.
