Cedar Creek Hospital is seeking a dynamic and talented Intake Clinician, to join our team of compassionate, dedicated professionals. The Intake Clinician, LMSW responds to requests for services and determines clinical appropriateness of each request. Information is normally obtained through initial clinical phone information, followed by clinical evaluation if appropriate for the client and coordinates with the physician regarding patient care needs. The Intake Clinician will facilitate this process by assisting the client, family and referral sources. They also provide information to clients, families and referral sources regarding treatment programs, insurance and admission procedures.
Qualifications
Intake Clinician Job Requirements:
LMSW licensed in the State of Michigan required.
Previous intake experience preferred.
