How You Will Make an Impact :
Develop relationships with residents and their families using a consistent caregiver approach, supporting physical, spiritual, emotional, and clinical needs.
Assess and evaluate resident clinical conditions, document findings in the electronic medical record, and notify providers and / or RNs when necessary.
Update or modify care plans as needed to reflect changes in resident care and service needs.
Supervise and lead care associates.
Administer medications timely, accurately, and according to orders.
Participate in meetings related to resident outcomes and care.
Respond to residents, family, or responsible parties in a timely and professional manner.
Participate in clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
Perform other duties assigned by the RN clinical leader.

What You Will Need :
Graduate of an approved Licensed Practical / Vocational Nursing program.
Minimum 1 year of experience in assisted living, home health, or long-term care preferred.
Passion for caring for seniors and being a strong resident advocate.
Experience with electronic medical records preferred.
