Cedar House Life Change Center (CHLCC) is a 125-bed treatment facility and 501(c)(3) non-profit corporation that was awarded T op Work Placed in 2021 . Our organization encourages our employees to grow in their positions and promotes them from within the company.

We are seeking a Care Coordinator to join our team to continue to provide high-quality Substance Abuse Treatment.

Salary range for this position is $24.00 - $32.00

What You'll Do :

Under the supervision of the Program Director, will assist in providing residential program services. Will cooperate with other staff personnel in the planning, evaluation, training, and implementation of a process for program efficiency and client care :

  • Coordinate with ancillary services, including individualized connection, referral, and linkages to community-based services and supports, including but not limited to educational, social, prevocational, vocational, housing, nutritional, criminal justice, transportation, childcare, child development, family / marriage education, cultural sources, and mutual aid support groups.
  • Discharge planning, including coordinating with SUD treatment providers to support transitions between levels of care and recovery resources, referrals to mental health providers, and referrals to primary or specialty medical providers.
  • Coordinate with medical and mental health care providers to monitor and support comorbid health conditions.
  • Document care coordination notes in EHR in 15-minute increments
  • Collaborate with other professionals within the agency to meet the needs of the beneficiary.
  • Collaborate with other professionals outside of the agency, such as San Bernardino County Care Coordinators, Riverside County Substance Use Community Action Resource Evaluation Services Case Managers, Mental Health Providers, Primary Care Physicians, and Medicated Assisted Treatment Providers, to address the needs of the beneficiary.
  • Arranging transportation services to and from appointments.
  • Participate in multidisciplinary treatment team meetings to assess the needs of the beneficiaries.
  • Maintain current CPR and First Aid certification.
  • Enforce all Cedar House rules and regulations at all times.
  • Safeguard resident's and visitor’s welfare at all times.
  • Pursue additional status / level advancement whenever possible.
  • Maintain current professional license / certification.
  • Continue to acquire and update the knowledge and skills necessary to address substance use disorders and related counseling issues.
  • Adhere to all policies and procedures of the organization.
  • And such other duties and / or responsibilities as are assigned.
  • Requirements : What You'll Bring

  • Be currently enrolled or have completed a drug studies certificate program.
  • Be CAADE or CCAPP certified / registered.
  • Have at least 1 year of experience working in a residential setting.
  • Have completed 12 years of education or GED.
  • Must understand the nature of alcohol / drug addiction and its recovery.

  • PHYSICAL REQUIREMENTS

    While performing the duties of this job, the employee must be able to remain in a stationary position 80% of the time. They will also need to move around inside the office occasionally to access filing cabinets, office machinery, etc. The employee must be capable of operating a computer and other office productivity machinery. There may be occasions where the employee needs to climb up or down a ladder or equipment and position themselves to maintain files in file cabinets. It is essential for the employee to communicate information and ideas clearly so that others can understand. They must also be able to provide accurate information in these situations. The employee should be able to observe details at close range and occasionally move boxes weighing up to 20 pounds for various reasons. Additionally, the employee may need to work around or be exposed to outdoor weather conditions.
