Summary : The Quality Coordinator will perform the quality assurance functions under the direction of the Director of Population Health and in accordance with company administration goals.

Responsibilities :
Interpret quality data.
Educate patients on preventative screenings.
Complete chart reviews and outreach to providers, staff, and patients about open gaps.
Facilitate timely and accurate reporting of data internally and to payors.
Provide reports on gaps closed.
Lead and educate employees on quality reports.
Assist in the development and implementation of quality initiatives.
Lead the practice to meet the requirements and goals in value-based care, Quality, and other regulatory programs as set by the Director of Population Health.
Accountable for the daily workflow to capture quality.
Oversee and manage assigned projects.
Serve as the quality point person.
Lead processes in conjunction with the clinic supervisor, to manage compliance with value-based metrics and specifications for quality.
Communicate quality standards and parameters to appropriate staff.
Coordinates the testing and advancement of internal quality processes.
Identify and analyze issues, bugs, defects, and other problems, particularly when problems occur in multiple locations; recommend and facilitate solutions to these issues.
Review patient and user feedback to assess process effectiveness and efficiency.
Maintain compliance with federal, state, local, and organizational laws, regulations, guidelines, and policies.
Support staff morale.
Support a positive work culture to align with our mission and values.
Perform other duties as assigned.

Education : High School Diploma or equivalent.
Experience :
Medical office, nursing home, or inpatient facility experience preferred.
Care management or population health experience / knowledge preferred.
Mental and Physical Requirements :
Varied activities including standing, walking, reaching, bending, and lifting. This position requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Must be able to use a variety of office equipment. May require working under stressful conditions.
