Cedar Sinai Park is looking for a Social Services Assistant!
We are located at 6125 SW Boundary St. Portland, Oregon 97221, in the Raleigh Hills area of southwest Portland. We are on bus line 54 and 56 from Portland to Beaverton.
Summary
Assists the Social Services Coordinator in facilitating admissions and discharges; performs ongoing assessment of resident needs; works with residents, families, and staff to promote resident well-being.
Essential Functions

· Assists the Social Services Coordinator and Resident Care Managers with the admission process, including but not limited to admission paperwork, identifying resident needs, and assisting residents and families with the transition process.
· Assists the Social Services Coordinator in developing a comprehensive psycho-social care plan and history of each resident as part of the admission process.
· Assists the Social Services Coordinator in assuring supportive services for residents, including but not limited to room moves, special requests, and resolving crisis situations.
· Assists interdisciplinary team in gathering information for appropriate discharge of a resident. Works with the resident, family, and community agencies to obtain equipment and services prior to discharge.
· Assists Resident Care Managers with the Minimum Data Set (MDS) process.
Required Skills/Abilities
Consistently delivers excellent quality of service. Maintains and demonstrates ethical conduct. Demonstrates ability to communicate with geriatric and/or confused residents and a variety of other people such as staff, family members, visitors, and private duty caregivers. Positive and constructive attitude. Demonstrates effective time management. Excellent listening skills.
Required Education, Experience, Licenses, and Certifications
· High school diploma or equivalent required. Associate’s degree preferred.
· Long Term Care, geriatrics, or health care experience not required. Working in a healthcare setting or previous training helpful/preferred.
· Valid Oregon driver's license required.
