What will you be doing in this role?
The Clinical Social Worker LCSW provides a full range of social work services in collaboration with other CSMC staff to meet the psychosocial and discharge planning needs of patients/families/significant others. Active participation in Department's continuing education/staff development programs. Involved in program planning in assigned service areas along with the organization of social work activities. Provides clinical social work services to patients and their families with psychosocial needs. To assure patients receive benefit from medical and/or psychiatric care and post-hospital planning. To be available to non-licensed staff as a clinical resource/mentor to assist with complex cases.
- Crisis intervention

- Interdisciplinary discussion of psychosocial implications of illness including consultation regarding treatments and resources designed to solve the problems of the patients
- Develops and maintains productive relationships with community health, welfare, and social agencies

Dept Specific Responsibilities:
· Point person coordinating all aspects / components of the trauma survivor network concepts including:
o Coordinating all aspects of the in-patient peer support visits
o Facilitating all aspects of the peer support meetings (virtual and in-person)
o Tracking and reporting select data points on the program including program evaluations
o Responsible for quality and reporting of all program elements
o All patient privacy compliance aspects of the program in collaboration with the appropriate departments
· Patient reported outcome measures: will develop, implement, and sustain a post discharge follow up program for the trauma patient population based on the best evidence available, and trauma hospital requirements (criteria). Specific duties include but are not limited to:
o Completing recovery assessments, determining necessary referrals, providing community referral resource information, psychosocial support as needed, and follow through / follow up calls/meetings for tracking and reporting purposes
o Maintains appropriate database elements for monitoring, performance improvement, regulatory requirements
· Subject matter expert for psychosocial support of our trauma patient population and the families of our trauma patients including but not limited to:
o Provides support to the Trauma Service, nursing, social work, clinical care coordinators
o Provides in-services to staff on related aspects of care
o Provides assistance and training staff to complete screening, brief interventions, and referrals (SBIR) for alcohol in trauma patients
o Provides assistance and training staff to complete screening for acute stress disorder (ASD) on in-patient population, and post traumatic stress disorder screening (PTSD) in the outpatient / post discharge patient population
· Attain and sustain Level I trauma hospital requirements specific to the psychosocial support of trauma patients and their families within the parameters of this job description. Specifically,
o Monitoring compliance with SBIR, ASD, PTSD, and depression
o Participates and collaborates in efforts to attain and sustain threshold compliance with these three screening requirements
o Monitors the State of California Trauma regulations, the LA County DHS-EMS Agency Legal trauma hospital contract, and the American College of Surgeons trauma hospital criteria for all social work, and psychosocial requirements; will work to collaboratively to implement new requirements, and disseminate information to all appropriate staff
· Quarterly reporting to Social Work department.
o Will attend department of social monthly staff meetings to provide status reports to the department on aspects/progress of the TSN
Will communicate with Manager of Social Work routinely and as needed
Education:
- Master's Degree
- Graduate of an accredited School of Social Work with a master’s degree
License/Certifications:
- LCSW Required
Experience:
- Three years in an acute care hospital setting required
- Must possess a minimum computer competency comprised of a working knowledge of Windows or comparable system (specifically including keyboarding and mouse skills). As applicable to individual job duties and expectations, employee must additionally possess a working knowledge of word processing, spreadsheets, presentation development, e-mail, browsers, and online reporting.
