Job Summary
The Prevention Family Partner will, in collaboration with the CEDARS Runaway and Homeless Youth (RHY) programming team, provide parenting education and support to parents under the age of 20 who are not currently living with their family of origin. The Prevention Family Partner will focus on providing high quality parenting education, assessment, intervention and referral to alleviate risk factors. Collaboratively with the CEDARS RHY team, services will be delivered through home visits and group educational opportunities.

Responsibilities
- Support a caseload of 12-14 families.
- Provide short-term (3-6 month) case management services to families.
- Maintain consistent records of all case management activities, including providing timely documentation of client contact, completion of required paperwork and assessments, and completion of collaborative documents.
- Participate in collaborative meetings and activities through the initiative.
- Provide consistent, collaborative, and professional communication with assigned schools and partner agencies.
- A bachelor’s degree in human services or a related field
- Experience in providing case management
- Valid Nebraska driver’s license and a good driving record.
- Knowledge of trauma informed care.
- A self-starter who can work independently
- A collaborative team member who enjoys building relationships with others
