Shift: 1st shift 8:00am-4:00pm
Job Description
- The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines
- Identify resident acuity and needs, help to determine specific care needs, and communicate needs/expectations to families and responsible parties
- Complete assessments, PDPM, and care plans for all residents assigned
- Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines
- Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
- Ensure accurate, timely completion of the Triggers sheet for assigned residents
- Initiate care plans and supporting activities that will result in the best possible outcome for assigned residents
- Monitor and ensure completion of 7-day look-back documentation by nursing staff within the prescribed time frame
- Maintain current knowledge of PDPM assessment process
- Run the Restorative program in the community
- Responsible for Quality Measure Program
- Other related management duties as directed

Qualifications
- Licensed RN in the State of Indiana
- One year previous experience in a long term care environment
- Certification in both MDS an Restorative
- Experience in Restorative Programming; fall care, monitoring, investigation, prevention
- Knowledge of Medicare and Medicaid reimbursement
