Position Summary
The Restorative Nurse is responsible for the development, implementation, monitoring and supervision of the restorative nursing program for the facility. The Restorative Nurse is the liaison between formalized therapy and nursing. The Restorative Nurse promotes a restorative nursing philosophy among all facility departments.
Job Description
- Reports to and is supervised by the Director of Nursing.
- The Restorative Nurse has direct authority and supervises the Restorative CNA’s (RNA’s).
- Restorative/Rehabilitation Nurse works closely with the nurses and floor CNAs to insure that the restorative plan of care is being followed, thereby promoting resident’s to their highest practicable level of functioning.
- The Restorative nurse shall review each resident’s need for restorative services and designate the necessary interventions based on the need.
- He/she assists to develop and encourage a teamwork approach within the facility as it pertains to clinical issues as well as the restorative nursing care needs of residents in the facility.
- Maintains a current list of resident’s participating in a restorative program on each unit. For each unit these lists will be specific as to ambulation, ROM, splints, walking and other restorative programs.
- Completes all necessary Restorative Forms on admission, readmission, or change of condition within OBRA Parameters.
- Creates care plans for each program, verifying that the steps for completion of the program/tasks are pushed to the kiosk for the RNA/CNAs to view. Complete care plan for restorative programs, at a minimum, on (re)admission, quarterly, annually or change in condition.
- Oversees that residents get formalized therapy, as indicted and that the necessary assistive feeding and/or ADL devices are obtained as recommended. Works with nursing and dietary to see that these assistive devices are consistently available and that they are addressed on the care plans.
- Works closely with the Skilled Therapy department. Oversees that the specific recommendations, when discharged from therapy are carried out.
- Makes rounds to oversee and monitor the programs and to provide 1:1 teaching, as necessary.
- Attends care plan conferences and makes recommendations. Reports on progress of restorative nursing programs.
- Works with staff in developing tools appropriate to perform QI audits. Participates in QAPI for restorative nursing. Reviews monthly/quarterly indicator/measure lists, along with eHDS and utilizes this information to problem solve.
- Develops and implements innovative and creative ways to keep staff focused and enthused about restorative nursing, ie contests, awards, themes etc.
- Monitors that equipment and supplies are available and that they are in good working order.
- Problem solves with staff in regards to issues in providing restorative nursing care.
- Assists with orientation of new staff regarding the role, responsibilities, principles and practices of restorative nursing.
- Actively participates in physical restraint reduction programs, and fall management programs.
- Act as a member of the facility fall committee; assisting with updating the fall care plans as necessary.
- Completes assigned areas of the MDS, including Care Area Assessments.

Qualifications
- RN/LPN in the State of Illinois.
- Rehab certified meeting the State Of Illinois 60 hour requirement.
- Knowledge and understanding of the Nursing Process.
- Ability to work with the interdisciplinary team.
- Knowledge and understanding of the MDS and PPS.
- Knowledge of nursing policies and procedures.
- Excellent written and verbal communication skills.
- Supervision and Leadership
- Must be able to read, speak and understand the English language.
