Job Summary :
We are seeking a compassionate and dedicated Medical Social Worker (MSW) to provide critical social work services to patients in their homes. Under the supervision of the Director of Clinical Services / Clinical Manager, the MSW will assess and address social and emotional factors that impact patient care, working closely with patients, families, physicians, and the care team to ensure comprehensive, compassionate care in accordance with the plan of care and physician orders.
Responsibilities :
- Adhere to established Agency policies and procedures, promoting a high standard of care.
- Understand and advocate for the principles of Quality Assessment and Performance Improvement (QAPI).
- Serve as a consultant to Agency personnel on social and emotional issues impacting patients.
- Collaborate with community resources to coordinate care, utilizing appropriate services and supports.
- Safeguard patient confidentiality and protect all personal health information.
- Provide expert social work consultation to employees, patients, physicians, and allied health professionals.
- Build and maintain effective working relationships with physicians and interdisciplinary teams.
- Maintain punctuality and regular attendance.
- Stay current with Medicare Conditions of Participation for social work services.
- Report any accidents, incidents, lost articles, or unusual occurrences immediately to the Director of Clinical Services / Clinical Manager.
- Participate in in-service and continuing education programs.
- Attend patient care conferences and provide input on the development and evaluation of care plans.
- Establish and maintain contact with community support groups, offering expertise as needed.
- Plan and coordinate all social service activities within the Agency.
- Document patient and family services in accordance with Agency policy.
- Assist physicians and the care team in identifying the social and emotional factors impacting health and recovery.
- Contribute to the development and ongoing revision of individualized care plans.
- Prepare detailed clinical and progress notes in accordance with Agency policies.
- Engage with patients and families to provide guidance, emotional support, and social services.
- Assist in discharge planning to ensure continuity of care and resources after leaving the agency’s services.
- Perform other duties as assigned by the Director of Clinical Services / Clinical Manager or supervising social worker.
