Responsibilities
- Lead and collaborate with patients, families, and healthcare professionals to develop and implement personalized care plans
- Provide management and delivery of patient care plans
- Maintain accurate and timely records of patient care
- Participate in on-call rotations and respond to urgent situations
Requirements
- RN license in the state where you work
- Graduate from an approved school of nursing
- One year of RN experience and clinical competence to deliver quality patient care
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
- Commitment to consistently meet critical deadlines for charting
- Skill needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones, and EMR software
