About the Role:
As a Blended Case Manager (BCM), you'll work closely with individuals and families to provide case management services and advocacy for those living with serious mental illness or emotional disturbance. Your efforts will help support individuals in maintaining stability within their home and community environments.
What You'll Do:
- Provide services across Cameron and Elk Counties (travel required; mileage/time reimbursed per policy)
- Work 40 hours per week for 40 weeks per year (some non-traditional hours may be required)
- Provide case management services to adults and children with serious mental illness or emotional disturbance
- Work with individuals and families to access, link, coordinate, and monitor needed services
- Provide support and advocacy for clients and their families
- Maintain client records and documentation
- Collaborate with other professionals and agencies to ensure comprehensive care
- Adapt work hours to the needs of the position, including occasional evenings, weekends, and on-call emergency services
What We're Looking For:
- Bachelor's Degree in a relevant field (e.g., sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education) or a registered nurse; or a high school diploma and 12 semester credit hours in a relevant field plus 2 years of direct contact experience with mental health individuals
- 1-3 years of experience working with children and/or adults in a mental health setting preferred
- 1-3 years of case management experience preferred
- Excellent verbal and written communication skills
- Valid driver's license and access to a reliable automobile
- Ability to work in various settings, including individuals' homes and community locations
- Strong organizational skills and ability to remain composed in pressured situations
- Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
