The SUD Case Manager provides direct mental health services to a variety of individuals which may include children/adolescents, adults and geriatrics.
CORE RESPONSIBILITIES
- SUD case managers will help address practical concerns such as housing, resource acquisition, access to healthcare and should be documented in treatment plan.
- SUD case managers will also play a critical role in monitoring client participation in treatment and provide updates on progress for compiling provider summaries or any other need to help facilitate successful treatment outcomes.
- Serves as a single point of contact in their region for all clients seeking MAT services and for referral parties such as probation, DHS, medical practices, etc., and facilitate client engagement into treatment.
- Serves as a community liaison for internal staff and community stakeholders and educate local communities about their experience in dealing with the ongoing opioid epidemic, dependence and treatment, as well as Centennial Mental Health’s MAT program that treat Opioid dependence and other substance use disorders.
- Monitor treatment protocols and compliance for all MAT clients in their region, serving as a liaison for the clinical team. Communicate to the treatment team when clients are not maintaining compliance in MAT program services. (It is up to the provider to make any necessary medication recommendations or changes that are recommended by the clinicians).
- Coordinate all internal and external services (Medical, Clinical, and Case Management); interface with local medical providers as needed and insure that community based resources are accessed as needed. Coordinate transitions in and out of the program to and from other levels of care.
- Provide ongoing resources for Special Women’s services and special connections populations.
- Provides supportive behavioral health interventions to clients either in individual or group session
- Develops, monitors, and follow up of client case management treatment plans
- Assist client’s with medication needs by collaborating with agency nursing team
- Collaborates with other Centennial service providers to engage clients in care
- Coordinates clients’ use of additional Centennial programs and outside community resources
- Provides services in a variety of settings
- Maintain accurate and timely clinical records consistent with Center standards
- Participates in meetings and in services
- Participates in supervision sessions, facilitate intra-agency communication for each case and lead weekly case staffing in group supervision with clinical and or SUD supervisor.
- Performs other job duties as assigned
- Demonstrates a commitment to continuous skill development to competently serve individuals from diverse cultures and backgrounds.
EDUCATION:
Bachelor’s degree in psychology, social work, or other human services field
