Monday - Friday 3

Assist with admission screening of potential patients to determine the level of care, services
required, equipment needs and insurance coverage
Assists in acceptance of patients and coordinate admissions with appropriate staff provides
all pertinent information to staff regarding the patient and their needs
Responsible for collecting and responding to all referral inquiries as directed whether in
person, on the phone, or via computer, to convert inquiries into potential admissions
Assist in interviewing residents/guardians and obtaining required information and signatures
Admit and prepare identification records for residents in accordance with established policies and
procedures. Obtain all necessary signatures
Communicate with all Case Managers for managed care patients as assigned
Provide requested clinical updates to Case Managers and necessary staff
Maintain an accurate record of available beds
Admit, transfer, and discharge residents in accordance with established policies and procedures.
Develop and maintain a good working rapport with inter-department personnel, as well as other
departments within the facility
Assist in recording all incidents/accidents. File in accordance with established policies and
procedures
Maintain a current listing of all residents, their level of care, and assigned room numbers and provide
copies of such listing to nursing service, administrative personnel, government agencies and
personnel, etc., as necessary
Perform administrative duties as assigned
Upload and disseminate data as required
Assure that work/assignment areas are clean and all confidential information is secured before leaving
the work area
Assist in developing procedures to ensure records are properly completed and uploaded
Notify proper departments when the resident has arrived at the facility to assure that a member of the
nursing staff escorts the resident to his/her assigned room.
Maintain various registries as directed including register for admission and discharge of residents
Maintain current records of all residents admitted/discharged by this facility in accordance with
established policies and procedures
Other related duties and responsibilities that may become necessary to assure that the needs of the
facility are met
Personnel Functions
Maintain an excellent working relationship within the Admissions team and cross
functionally
Report occupational exposures of blood, body fluids, infectious materials, and hazardous
chemicals in accordance with the facilitys policies and procedures governing accidents and
incidents
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as
well as a calm environment throughout the facility
Staff Development
Attend and participate in continuing educational programs and in-services designed to keep
you abreast of changes in your profession and Company policies and updates
Attend meetings and in-services as required
Complete assigned Relias training timely
Safety and Sanitation
Report all unsafe/hazardous conditions, defective equipment, etc., to your supervisor immediately
Equipment and Supply Functions
Ensure supplies have been replenished in work areas as necessary
Ensure equipment is in working order

Be sure that appropriate protective clothing/devices are on hand and used for handling
infectious waste and/or blood/body fluids
This position is not a remote position onsite presence is required during all working hours
Working Conditions
Works in office areas as well as throughout the facility
Moves intermittently during working hours
Is subject to frequent interruptions
Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster,
etc.)
Is involved with residents, family members, personnel, visitors, government agencies/personnel, etc
Is subject to hostile and emotionally upset residents, family members, personnel, visitors, etc.
Is involved in community/civic health matters/projects.
Is subject to exposure to infectious waste, diseases, conditions, etc
Communicates with the medical staff, nursing personnel, and other department supervisors
Maintains a liaison with the residents, their families, support personnel, etc., to assure that the
residents needs are continually met
May be subject to the handling of and exposure to hazardous chemicals.

Education
Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of
an approved LPN/LVN or RN program.
Experience
None. On-the-job training provided.
Specific Requirements
Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN in this state.
Must be able to read, write, speak, and understand the English language.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors,
government agencies/personnel and the general public.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations,
and guidelines that pertain to long-term care.
Must possess leadership and supervisory ability and the willingness to work harmoniously with and
supervise other personnel.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals,
objectives, policies and procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle
difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into
existing nursing practices.
Must be able to relate information concerning a residents health.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices)
Must be able to move intermittently throughout the work day.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.

Must be able to see and hear or use prosthetics that will enable these senses to function adequately to
ensure that the requirements of this position can be fully met.
Must function independently and have flexibility, personal integrity, and the ability to work
effectively with residents, personnel and support agencies.
Must meet the general health requirements set forth by the policies of this facility which include a
medical and physical examination.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile
people within the facility.
Must be able to lift up to 20 pounds. Requires infrequent lifting of more weight up to 50 pounds and
up to 100 pounds with assistance. Frequently pushes and pulls carts and transports residents.
May be necessary to assist in the evacuation of residents during emergency situations.
