Job title Counselor II

Reports to Outpatient / Residential Supervisor

Job purpose

Provide individual and group counseling, treatment planning, and assessment under the supervisory report of the Outpatient / Residential Supervisor.

Qualifications

Licensed by the Ohio Chemical Dependency Professionals Board (LCDC II minimum) or the Ohio Counselor, Social Worker, Marriage and Family Therapist Board. A minimum of one (1) year paid experience working in counseling is preferred. Previous experience in electronic charting is helpful.

Duties and responsibilities

  • Provide therapeutic treatment services to adults with substance use disorders and / or other co-occurring mental illness, under the direction of their clinical

supervisor and within their scope of practice.

  • Provide individual, couples, family and group services as assigne d
  • Complete all required documentation - assessment, treatment plan, progress notes, and transfer / discharge notes.
  • Excellent clinical skills in order to diagnose and treat DSM 5 mental health disorders (under supervision), including substance use disorders that is within their
  • scope and professional competencies.

  • Coordinate all aspects of care with other members of the interdisciplinary team.
  • Maintain at least an overall score of 90% in all areas of chart review.
  • Participate on committees and in quality improvement activities as assigned.
  • Maintain credentialing or licensure necessary for the position and continually update professional skills through in-service training and other sources.

  • Patient Rights- Adheres to all patient rights standards as well as serves as a resource to patients regarding those rights.
  • Professional- Exhibits professional behaviors such as maintaining the CAT Code of Ethics, working cooperatively with others following CAT personnel polices etc.
