Position Summary:
Provides case management services to eligible consumers under the Pennsylvania DPW Code Title 55 Base Service Unit Service Description.

Essential Functions:
- Ensure continuity of care and service connection
- Complete all documentation and data entry in a timely and accurate fashion.
- Maintain utilization requirements.
- Provide follow up to identified consumers
- Be well organized, have excellent verbal and written communication skills.
- Computer competency with the ability to learn agency programs.
- Other duties as assigned to achieve the mission of the agency.
- Conduct a comprehensive screening to determine mental health service needs
- Provide Information and Referral
- Completes registration process and scheduling of all Mental Health Assessments.
Daylight, Monday-Friday hours. Some flex schedule and remote work after onboarding! No weekends, No on-call, and No holidays! Travel required. Mileage reimbursement is available at the state set rate!
The hourly starting rate is $17.95. Depending on experience and education may start higher.
Qualifications:
Bachelor’s degree in human services or related field. One-year mental health or related experience.
Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
