Position Summary:
The Housing Specialist/Veteran Housing Advisor will Provide case management services to eligible consumers under the Pennsylvania DPW Title 55 Base Service Unit Service Description and the Supportive Services to Veteran Families (SSVF) program to provide outreach, assessment, and case management to all referred veterans and others who are experiencing homelessness or near homelessness.
Essential Functions:
- Conduct comprehensive assessments and develop a treatment plan for consumers to determine needs and resolve barriers.
- Outreach to all providers, agencies, and entities to education and inform of services provided by the SSVF program and to solicit referrals to program.
- Screening of all referred veteran individuals/families to determine eligibility for the program
- Case management for all eligible veterans and families, including housing-focused case management to ensure stable housing plan
- Connecting all eligible Veterans and families to appropriate VA resources/benefits
- Connecting all eligible consumers to mainstream resources and benefits based on the assessment of needs
- Coordinate and locate housing services.
- Meet with consumers at least once a month but may be more depending on established goals and needs.
- Enter and update HMIS at least once per month or within 7 days of change.
- Conduct homelessness outreach on a re-occurring basis.
- Conduct and document monthly inspections for housing programs and provide follow-up with needed services or supports.
- Organize and assist housing program participants with moving during program enrollment and discharge/termination.
- Collaborate with other housing program staff for needed life skills.
- Ensure continuity of the services for the consumer and coordinate the services received.
- Complete all reports, documentation and data entry in a timely and accurate fashion.
- Maintain utilization requirements.
- Facilitate SOAR applications.
- Provide follow-up to identified consumers.
- Other duties as assigned to fulfill the mission of the agency.
Qualifications:
Must have Bachelor Degree in Psychology or social work or a 4 year degree with 18 credits in psychology. One-year experience in social services field, experience in working with homeless population and computer literacy. Preferred HMIS database entry knowledge of housing first.
Valid driver's license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
