Responsibilities:
- Support individuals with disabilities in achieving their goals and living independently.
- Collaborate with interdisciplinary teams to develop personalized care plans.
- Provide training and guidance on basic activities of daily living, self-care, and household management.
- Monitor and report on individual progress, making adjustments to care plans as needed.
Requirements:
- High school diploma or equivalent required; one year of related experience preferred.
- Must be at least 18 years old and able to lift a minimum of 50 pounds.
