The Center for Extended Care and Rehabilitation at Amherst, a 134-bed skilled nursing and rehabilitation center, has been providing exceptional Short Stay and skilled nursing care to adults and elders for over 50 years and has some very exciting job opportunities to offer you.
Purpose of your Position :
Assists in the provision of screens, collection of assessment data, and direct treatment to a general rehab caseload under the direction and supervision of a Physical Therapist. Participates in the clinical team process. Provides teaching and training to caregiver staff. Participates in clinical program development.
Major Duties and Responsibilities :
Screens patients / residents.
May perform additional assigned administrative duties such as : assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
Under a physician’s order provides direct resident treatments in accordance with an established plan of care. Treatment venue includes individual, supervised, and group.
Contributes to the team meeting and decision making process as defined by the program.
Consults with and trains patients, residents and caregivers.
Monitors efficiencies on an ongoing basis in collaboration with supervisor.
Documents activities according to the company’s clinical documentation system on a timely basis.
