CFCE expanded in early 2012 to include a health care center, The Pediatric & Family Health and Wellness Center.The Pediatric & Family Health and Wellness Center, a Federally Qualified Health Center (FQHC), offers an array of preventative health services including but not limited to : general and sick care for adults and children, OB / GYN, dental services, nutrition programs and mental health and substance abuse treatment for children and adults.As a Community Health Worker, you are a frontline public health worker who performs a wide variety of duties at the paraprofessional level to engage, support, advocate and educate community members in overall health and wellness.Why join CFCE : Great benefits package, including a Zero (0) cost out of pocket medical plan, 13 Paid Holidays and a competitive Paid Time Off PackageMaking an invaluable impact in your communityGrowth and professional development opportunities availableQualify for Public Service Loan ForgivenessWe are a tax-exempt organization under section 501(c)(3) of the Internal Revenue CodeSome of the Functions Include : Provide HIV / STI testing & counseling, education, and referral services to patients of CFCE and to the general public through community events, outreach, and other collaborative agencies.Provide prevention interventions, education, and resources relative to sexually transmitted infections within CFCE and through community events, outreach, and off-site presentations.Provide accurate HIV / AIDS & STI (and other testing) data collection, tracking, and reporting within CFCE and to required health agencies (Health Dept, CDC, HRSA, etc.)Assist HIV Project Coordinator with procedures, referrals, and patient contacting as needed / required.Educate patients regarding diagnosis, medications, treatments, and testingConduct outreach to identify and engage community members around health and wellness activitiesMinimum Education / ExperienceHigh School / GED requiredAA PreferredHIV / AIDS 500 / 501 Certified (Basics / Prevention of Counseling, Testing, and Linkage) (Preferred)Ora Quick rapid testing CertifiedAt least one-year experience in providing outreach and supportive case management services to at-risk, indigent, and / or homeless population (advocacy, housing placement, etc.), preferably working with HIV population and / or population(s) at high risk for HIV infection.Knowledge of local communities with high population of persons at risk for HIV and STI'sAbility to facilitate small groups to facilitate behavior changeEffectively communicate in writing in order to document patient information and progressCFCE is a Drug Free Workplace and an Equal Opportunity Employer.
