We currently have an open position for a Behavioral Health Consultant. This is a salary, exempt full-time position.
The Center for Family Health is a Federally Qualified Health Center (FQHC) located in Jackson, Michigan. We are a multi-specialty clinic with practitioners specializing in the areas of Family Medicine, Internal Medicine, Pediatrics, and

Position Summary:
The Behavioral Health Consultant is responsible for managing cases and providing behavioral health services to CFH patients. These services include individual and group counseling along with care management. These behavioral health services help improve our patient's health status and quality of life.
The Behavioral Health Consultant is expected to work in a team-based environment where the approach is a whole person care approach that engages the patient in their health care and requires all team members to be effective and supportive communicators. A high degree of integration and collaboration is required to meet the varying and changing needs of our patients.
Essential Duties and Responsibilities:

- Assist primary health care providers in recognizing and treating mental health disorders and psychosocial problems.
- Provide therapy services in individual, family, and group sessions as well as crisis intervention.
- Complete and document mental health screening and assessment.
- Develop a treatment plan with the patient's participation and continually assess for appropriateness.
- Provide clinically appropriate treatment based on mental health assessment and treatment plans.
- Work cooperatively with primary care team members and the family in the provision of services to meet the goals developed in the Treatment Plan.
- Complete timely and accurate documentation of all therapy and health coaching sessions, significant incident occurrences, and phone conversations.
- Provide assessment, brief intervention, consultation, and education with an emphasis on behavioral and lifestyle management through use of clinical and motivational interviewing skills.
- Supports patients in developing self-management skills and in adopting positive behavior changes.
- Identifies and addresses barriers to patient's ability to follow medical treatment plan.
- Refer for additional or alternative services, when indicated, and assist patients to arrange for services to follow through on recommendations.
- Lead patient education and support groups.
- Works collaboratively with clinical staff on pertinent issues and coordinates patient care.
- Participate in peer review by performing peer audits and meeting audit requirements.
- Participate and engage in case consultation meetings.
- Engage in QI initiatives.
- Conform with and abide by all regulations, policies, work practices and instructions.
Education/Training/Experience:
- Must have master's in social work (with limited or full license), or hold a licensed professional counselor license.
- Minimum of 2 years' experience in Social Work preferred.
- Must have BLS certification or the ability to attain one.
Job Knowledge, Skills & Abilities:
- Ability to design and implement clinical pathways and protocols for treatment of selected mental and behavioral conditions.
- Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
- High level of comfort in working with primary care medical providers using a consultative style.
- Ability to make quick and accurate clinical assessments of mental and behavioral conditions.
- Ability to work effectively both independently and in a team environment.
- Excellent communication skills, both written and verbal.
