Case Manager

Center For Family Outreach (TFFO) is hiring a Case Manager to fulfill its mission of strengthening families by providing education, prevention, and early intervention for substance use through assessments and therapy, emotional, mental health, and wellbeing counselling, conflict resolution and healing and navigating the teen years with parent resources. Case Manager may also provide Parent Support and teach parenting classes. Case Managers will also work closely with youth in the Teen Activity Center.

POSITION SUMMARY

The Case Manager serves as the primary contact while youth and family are enrolled in the program. The Case Manager provides youth and family resources, advising, case management, instruction, academic monitoring, mental/behavioral health issues and substance monitoring in both Fort Collins and Loveland. The Case Manager also assesses, plans, implements, monitors and evaluates progress while youth and family are enrolled in the program.

ESSENTIAL DUTIES & RESPONSIBILITIES Other duties may be assigned

  • Provide education, advising, and unconditional acceptance, active listening and offering of coping mechanisms
  • Provide the youth with the support and advocacy they need to survive their crisis and begin their long-term healing process.
  • Identify and refer youth to community resources and different types of governmental assistance available for families experiencing crises.
  • Monitor student compliance in school and program non-compliance; determine appropriate course of action
  • Provide parents with resources and teach parenting classes as needed.
  • Coordinate and collect data for grants and other reports
  • Accurately and consistently enter data into agency spreadsheets and databases for tracking of outcomes
  • Assess strengths and needs of youth and families and develop strategies to ensure safe environment
  • Works with multiple county and city, school district and law enforcement agencies
  • Develop, instruct and evaluate educational classes regarding drug and alcohol, communication, and life skills.
  • Conduct pre/post-test assessments
  • Consult with the Executive Director on strategies for managing difficult/complex cases
  • Attend seminars, conferences, workshops, classes, lectures, etc., as appropriate, to enhance and maintain knowledge of trends and developments in field; review professional journals, attend association and professional meetings; and otherwise maintain contacts with professionals to facilitate exchange of information
  • Consult with the Executive Director before making a child protection report
  • Coordinate class scheduling, youth enrollment and appointments

MINIMUM QUALIFICATIONS

Education and Work Experience

· A Bachelor's degree in a human development, criminal justice or secondary education and two years related experience or equivalent combination of education and experience.

· Knowledge or experience with at-risk children and youth, including knowledge/experience with mental health, trauma, or substance abuse issues

· Able to use principles, practices, and objectives as related to case management, education, risk assessment, and family crisis intervention techniques.

· Entry level case management techniques including: knowledge of family dynamics, motivational interviewing approaches, influence in changing negative behaviors

· Scope and application of laws (Colorado Children’s Code, Volume VII) and regulations pertaining to physical abuse, sexual abuse, neglect, and child welfare.

· Proficient in Microsoft Office Suite applications (Word, Excel, PowerPoint)

· Possession of a valid Colorado Driver’s License

The Center for Family Outreach was established in December of 2000 as a non-profit organization offering prevention, education and early intervention services for youth ages 11 to 18 and their families. We strive to restore hope and provide opportunities as we strengthen and inspire the youth of Larimer County and their families.

Job Type: Full-time

Pay: From $21.00 per hour
