Position Summary:
This position is responsible for providing services and tools to ensure self-sufficiency including case management, advocacy, and crisis-intervention for the GC HOME APR TBRA (rental assistance) This full-time, non-exempt position reports to the Housing Supervisor.

Duties and Responsibilities:
Assist participants with a service plan and housing stability plan to include education, employment, long term housing, social services, and any additional services-based participants needs.
Conduct screening in and intakes with participants.
Assist participants with assessing needs including but not limited to safety planning, home management, education/career opportunities, community resources and financial empowerment.
Connect participants with outside programs/agencies ensuring they have services to meet any challenges or barriers.
Connect client with a domestic/sexual violence, substance abuse, behavioral/mental health, health care and other needed services.
Any duties related to ensuring the participants have needed tools for success to transition to affordable housing within 24 months.
Maintain client files, following state and agency guidelines.
Complete statistics and reports.
Participate in meetings and trainings necessary for the success of the program.
Assist with other duties for the success of participants and program.
REQUIREMENTS:
The HOME APR Case Manager must successfully complete the 60-hour Sexual and Domestic Violence Training provided by Services Empowering Rights of Victims. The Case Manager must have experience working with unvenerable populations. Prior experience working with advocacy and case management is preferred. Effective verbal and written communication skills is required. Bilingual in English and Spanish is preferred. This position requires a Bachelor's degree in social work, criminal justice or another human service-related field.
