Center for Human Development, Inc. is seeking a full-time Child Case Manager providing skills training and case management services to individuals and their support system and/or family, in the school and community setting. Works with the child and family treatment team to provide goal-oriented services. Duties include supporting completion of intake packets with parents/guardians, coaching and mentoring in pro-social activities, life skills, communication, problem solving, behavioral management, and parenting skills. Co-Facilitate multi family groups. This position functions as a resource brokerage, helping connect students and families to necessary resources within the community such as food, clothing, healthcare, and other identified needs.
- Provide education to family members through support/skills building education groups.
- Assists clients and family in accessing community resources.
- Monitors client appointments and progress; maintains accurate case notes, follows treatment plans; completes reports as required in a timely manner.
- Provides supportive and problem-solving counseling to clients and their families; responds to crisis situations.
- Coordinate with school, regional children's programs and lead groups.
- Communicate / consult with schools re: student needs for support and other resources.
- Communicate/consult with school staff: regarding school staff needs for support and other resources.
- Participate in IEP or school team meetings when requested.
- Must have the ability to maintain positive and professional working relationships with co-workers, clients and other agencies.
- Meet with clients and/or family members on an individual basis to offer support, guidance, and insight into parenting skills and behavior management techniques.
- Implement activities according to individual treatment plans.
- Document all program activities in a timely manner.
The Requirement
- Bachelor’s degree with trainings in human development, human services or; a combination of at least three year's relevant work, education, training or experience in a mental health field with approval from Mental Health Director.
- Ability to understand and implement instructional methods while demonstrating flexibility, creativity and autonomy.
- Ability to maintain accurate and timely records.
- Ability to work with team, other professionals and lay persons.
- Willingness to learn through in-service and outside trainings.
- Knowledge of interviewing and counseling / treatment techniques.
- Knowledge of parenting skills / interventions.
- Knowledge of ADHD and other child behavior disorders, including psychosis.
- Good understanding of child development.
- Ability to apply behavior management skills.
- Familiarity with community resources for client and family needs.
- General demonstrated ability to cooperate, coordinate and accept supervision.
- Must possess a valid Oregon Driver’s license and be insurable for client service purposes, travel between business offices, and to attend required meeting and/or training.
- Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200-407-007-0370 and drug screening.
