Center for Human Development, Inc. is seeking a full-time Youth Case Manager and Care Coordinator responsible for planning, organizing, and coordinating the youth Wraparound program. Responsible for promoting community awareness of program, facilitating client services, and coordination of care. Engage families in intake and orientation process, provide coordination of care, identify barriers to family success, develop strategies to overcome, and facilitate out of home placement, as appropriate. Establish partnerships with local community members, schools, physical health, alcohol and drug treatment providers, and other organization or individual necessary to effectively coordinate the team-based treatment process. Partnering with local business and agency administrators to develop resources for delivering community-based services and promote community awareness of issue specific to youth risk factors. Collect and submit data per program guidelines.
The Requirements:
- Bachelor’s degree with trainings in human development, human services or; a combination of at least three year's relevant work, education, training or experience in a mental health field with approval from Mental Health Director.
- Understanding of mental health assessment, treatment and service terminology and ability to apply the concepts, provide psychosocial skills development, and implement interventions.
- Ability to understand and implement instructional methods while demonstrating flexibility, creativity and autonomy.
- Familiarity with community and regional resources and systems of care.
- Excellent communication skills including public speaking, interviewing and counseling skills.
- Ability to work well with others from a variety of disciplines, ease of manner, and conflict management skills.
- Organizational skills.
- Knowledge of state and local systems and knowledge of youth care intensive treatment methods and practice.
- Flexible schedule to include evenings and ability to travel for trainings.
- Must possess a valid Oregon Driver’s license and be insurable for client service purposes, travel between business offices, and to attend required meeting and/or trainings.
- Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200-407-007-0370 and drug screening.
Essential Functions:
- Provide education to clients and/or family members through skills building or educational groups.
- Assists clients and family in accessing community resources.
- Engage families in intake and orientation process.
- Facilitate meetings and coordination of care.
- Develop Proactive Safety Plans and implement service coordination plans.
- Establish partnerships with local community members, schools, physical health and alcohol and drug treatment providers and other organizations necessary to promote and establish youth care program activities.
- Identify family barriers and develop strategies for the elimination of identified barriers.
- Facilitate community resource team meetings and develop behavior support and safety plan and implement service coordination plan.
- Collect data and report on the state website.
- Complete foundational training within 90 days of hire.
- Maintain appropriate certifications required for position.
- Maintain awareness of funding available and other community resources.
- Participate in individual and group clinical supervision.
- Learn and comply will all applicable state laws related to Wraparound coordination.
- Must maintain punctual and regular work attendance.
- Must have the ability to maintain positive and professional working relationships with co-workers, clients and other agencies.
