What are the responsibilities and job description for the Direct Care - Community Based Day Services position at Center for Human Development?
The Center for Human Development, (CHD) is seeking Direct Care Staff to join our team at our Community Based Day Services (CBDS) Hawthorn Adult Day Health program located in Chicopee, MA. The Hawthorn program is dedicated to helping people live life to the fullest. We provide medical services, plan social activities, or simply offer supervision so the regular caregivers of the individuals we serve can attend to other responsibilities.
Your role as a Direct Care Staff:
As a Direct Care team member you will be supporting client's activities of daily living, integrating them in the community and promoting independence, providing nutritional support, administer medication as needed, help clients in areas like life-skills, money management, job development, self-advocacy and problem solving and regular outings like a trip to the park, library or nearby shopping centers.
Requirements Include:
- High School diploma or GED required
- Experience in the Human Services field is a plus.
- Ability to pass certification for CPR, First Aid and Safety Care.
- Ability to pass the Medication Administration Program (MAP)
- Reliable, registered & insured vehicle and approved driving record check.
- Approved criminal background check (CORI)

Shift available: Full time opportunity, Monday – Friday 8:00am-4:00pm.
