What are the responsibilities and job description for the Enrollment Specialist (TEMP to Hire) position at Center for Human Development?
The Center for Human Development (CHD), is currently seeking a temp-to-hire, Enrollment Specialist to join CHD’s Innovative Care Partners (ICP), located in West Springfield, MA. To meet organization needs, we will be hiring on a temporary basis, and there is opportunity that temporary assignment can lead to permanent hire.
ICP is a partnership with two other agencies that combine our expertise in behavioral healthcare.
YOUR ROLE AS AN ENROLLMENT SPECIALIST
The Enrollment Specialist will review the Electronic Health Record (HER) portal throughout the day. You will make calls to enrollees referred by MassHealth, MCO/ACOs and community providers to ensure that enrollees are assigned a Care Coordinator within five business days. Additional duties include, but are not limited to:
- Verifies enrollee eligibility for CP services with MassHealth.
- Utilizes various avenues including collateral contacts in order to obtain updated or confirm contact information for enrollee.
- Initiates recruitment contact to enrollees, by either phone calls, text/emails, mailing letters or conducting outreach visits to enrollees’ homes or community settings.
- Introduces enrollee to the ICP program.
- Provides enrollee a handbook and has enrollee review, understand and sign participation form and any other Community Partner program forms during enrollment.
- Uses the enrollment care coordinator availability list to assign enrollees to appropriate teams and care coordinators.
- Schedules initial visit for Care Coordinator.
REQUIRMENTS:
- High School diploma or equivalency
- Prior administrative office, customer service, or social service experience
- Excellent customer service and communication skills
- Bilingual capacity (Spanish/English) strongly preferred
- Valid Driver’s License
- Position is subject background check to include, CORI, Motor vehicle record and OIG check.
SUCCESS FACTORS:
- Strong Communication Skills for outreach to various communities and agencies throughout Western Mass to assist with enrollment.
- Engaging enrollees assigned to ICP.
- Knowledge of Mass Health’s Community Partners program and being able to talk about it to enrollees and members of the community.
- Ability to work with people from diverse backgrounds.
- Organized and efficient, detailed oriented.
- Computer proficiency and experience with electronic health records.
Pay rate is $20.00/hr.
Worksite details: You will work onsite at our West Springfield, MA location. Some travel required, use of personal vehicle required. Mileage reimbursement provided. (There are not options for Hybrid or Remote schedules for this role).
