What are the responsibilities and job description for the Integration Coordinator position at Center for Human Development?
The Center for Human Development, (CHD) is seeking an Integration Coordinator to join our Diversion, Shelter and Housing Services division. The Integration Coordinator provides consultation and training support to Diversion Shelter & Housing staff in order to increase awareness of available service options, referral processes, and follow through of (deliverables).
Your role as an Integration Coordinator :
The Integration Coordinator works on behalf of the Center for Human Development (CHD), Diversion, Shelter & Housing service line and collaborates with internal services across the service CHD continuum, community providers, as well as (relevant / associated) funding sources to ensure ease of access for participants. They coordinate communication with families, DSH team members, CHD leadership, and external providers, to identify, refer, and track, referral processes for participant referrals.
REQUIRMENTS :
- Must have a vehicle for work use and current driver's license
- All candidates must complete and pass background record screening process
- Previous experience with data management
- Awareness of available community resources and a commitment to supporting access to these resources
- Excellent communication skills with participants, providers, and teams
- Three years human services experience, preferably with the unhoused population
SUCCESS FACTORS :
Ideal candidates will have excellent organization skills, be meticulous with details to ensure a seamless process from start to finish in providing service deliverables to individuals and families alike in an ethical, (professionally informed), and timely manner.
Additionally some key skills will be :
Pay rate is $23.00 / hr. along with a benefit package that includes Dental, Health and Life insurance. Paid time off, earned vacation time and paid holidays just to name a few.
This is an onsite, 40hrs / week Full time opportunity.
