What are the responsibilities and job description for the Psychiatric Mental Health Nurse Practitioner position at Center for Human Development?
Seeking Full-Time or Part-time Psychiatric Mental Health Nurse Practitioners at multiple locations for CHD's expanding outpatient continuum of services. CHD's Outpatient services include but are not limited to individual counseling, psychiatry, primary care, crisis services, recovery coaching, case management, treatment for substance use disorders, and peer supports. Locations : Greenfield MA, East Longmeadow MA, Springfield MA, Holyoke MA, Easthampton MA, Orange MA, Chicopee MA.
Essential duties for these roles include :
- Provide outpatient psychiatric care including psychiatric evaluation(s), assessment, diagnosis and treatment of adults / youth
- Collaboration with physicians, advance practice nurses, psychologists, social workers, nurses and other professionals
- Provide clinical consultation, in person or remotely, to other member of the clinical team
- Collaborate with guardians, relatives, and significant others of people served when clinically indicated
- Counsel people served and their families when clinically indicated
- Document evaluations, assessments, diagnosis and treatments in the electronic health records
- Participate in individualized treatment plan designs
- Order, review, and interpret laboratory and other diagnostic tests
