What is a Patient Care Navigator?
A patient care navigator, also known as a care coordinator, is the health professional who ensures that patients get the best care possible. They listen to patients and their families to ensure needs are being addressed, educate patients on medical procedures or conditions, keep patient records up to date, and act as a liaison between the patient and healthcare personnel by keeping the lines of communication flowing.
Job Responsibilities of a Patient Care Navigator:
- Perform analytical and data entry task
- Answer patient calls, emails and questions
- Confidentially manage patient accounts
- Schedule patient visits and answer pre-visit questions.
- Ensure compliance with professional standards & regulatory requirements
- Provide patient consultations and recommended treatments
- Work as a team player to ensure each patient receives the best service possible
- Scan documents into patient charts
- Meet with patients and families regarding treatments, procedures, medications, and continuing care.
- Assist care team with setting goals for quality assurance and best practices
- Provide community resources and referrals for continuing care
- Supporting the Care Navigators (Educators)
Requirements:
- High school diploma or equivalent; Associates degree preferred
- 3-5 years of experience in a customer service and/or administrative role
- Proficient in Microsoft Office
- Previous clinic experience preferred
- Ability to go up and down stairs and lift up to 50 pounds
