The Center for Transitional Living is looking for a Certified Mental Health Counselor to join our exciting Agency that has just celebrated 10 years in business! We are seeking someone from the greater Hartford, CT area that can service our clients near Northern and Central Connecticut and other areas of the state.
MENTAL HEALTH COUNSELOR
Summary / Objective
The Mental Health Counselor position is directly responsible for the processes surrounding a
client’s growth based upon the individual’s treatment plan. This position requires 1 on 1
individual services to help ensure independent living is maintained.
Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the
essential functions.
Perform 1 on 1 counseling and or psychosocial therapy sessions with individuals
impacted by mental health challenges
Create and maintain appropriate client files
Authorizations, including but not limited to :
10 day and 60 day follow up documentation
Creating and confirming client schedules
Monthly notes
treatment plans
6 month reviews
Assist with further outreach for client’s needs based on Treatment Plan and
authorization clearance.
Maintain caseload and follow assigned visits per month
