What are the responsibilities and job description for the Early AM Case Manager position at CENTER FOR URBAN COMMUNITY SVC?
The Center for Urban Community Services, a national leader in the development of effective housing and service initiatives for homeless people, invites applications for an early morning case management position at the Street Outreach Program. CUCS is the lead support agency in the Manhattan Outreach Consortium (MOC) that uses the Housing First model. MOC is a coalition of neighborhood-based organizations working together to permanently house homeless people living on Manhattan streets.
CUCS is a leader in implementing new practices, sharing knowledge and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community.
The Early AM Case Manager is responsible for working with an assigned group of clients to help them obtain housing, help clients live full and satisfying lives in the community, help their team and program to function well and meet their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs’ core purpose, core values and vision statements. This role also entails daily canvassing of CUCS' catchment area (110th Street to the Northern tip of Manhattan) on foot and via agency vehicle.
A valid drivers license is required.
This shift is from 5:30am-1:30pm Monday-Friday.
Four overnight shifts per year are required for quarterly counts.
