Job Summary
Program Directors in the Day Habilitation Programs are responsible for the day-to-day operations of the program. DH Directors supervise Direct Care and Case Management Level Staff. Program Directors are responsible to ensuring that Program Member’s needs are met by organizing and managing schedules of trainings and scheduling staff. Program Directors serve as members of the Interdisciplinary Team and are involved in intakes and admissions to the program, development of Day Hab Service Plan and the ISP Process for program members. It is the Agency’s expectation that the Program Director be a visible presence within the program and exemplifies the Agency’s peer philosophy, working directly with Program Members and Day Hab Program Staff
Essential Functions:
· To be familiar with all aspects of regulations, certification requirements and policies that affect the industry standard and ensure their implementation in the program.
· Monitor accurate and up-to-date petty cash and Program Participant Community Access accounts by conducting regular audits.
· Order and maintain appropriate supplies in coordination with the Administrator, RN, Kitchen Staff or other members of the staff. Utilize central purchasing systems.
· To develop and provide staff with contextual training, scheduling and oversight or programmatic operations in cooperation with the Program Administrator, RN or other members of the ID Team.
· Responsible for assisting in the creation of and implementation of all aspects of the implementation of individual service plans (ISP), assessments, Day Hab Plans, measurable goals, tracking sheets and all data tracking requirements.
· Work in conjunction with the ID Team and the Clinical Department in communicating need, developing and implementing behavioral interventions specific to each Participant exhibiting concerns.
· Work collaboratively with Vocational Staff, Volunteerism staff and the Clinical Department to ensure that member’s needs are met.
· Develop monthly calendar for program members that include menu, trips, free activities that are to be submitted to the Director of the Program for review prior to distribution.
· Review and coordinate the weekly tracking of individual goals, objectives, programmatic opportunities and case notes to ensure timeliness, accuracy and neutrality.
· Complete agency paperwork as required including tracking of member attendance, monitoring staff attendance, providing information for Assurance Reviews, DMA Audits and CARF Certification.
The qualification listed below must support the essential functions and serve as the primary criteria for selecting candidates.
Minimum Qualifications
Bachelor’s Degree in Human Services preferred AND Two years of related Supervisory or Managerial experience in a Healthcare setting.
To meet all required background checks and drug testing upon hire and annually, as required by Regulatory Authorities.
