Job Summary
Center Point DAAC is seeking a compassionate and experienced Outreach Specialist to join our MAT Services team. As an Outreach Specialist, you will play a vital role in connecting individuals struggling with substance use disorders with our Medication-Assisted Treatment (MAT) program and other community resources. Your outreach efforts will focus on the surrounding areas of Santa Rosa in Sonoma County, with the goal of reducing barriers to treatment and promoting recovery.
Key Responsibilities
- Conduct educational presentations and outreach activities with local agencies, community providers, and potential participants
- Provide educational information about MAT and available program services
- Screen potential participants in various settings, including jails, prisons, and social service providers
- Assist clients in developing needs and service plans
- Complete accurate and timely documentation, including charting and progress notes
- Coordinate referral services with the coordinator and other team members
- Connect clients with community resources, including primary care, mental health services, social services, housing, and vocational services
- Assist with follow-up activities and make referrals for continuing care
- Provide other services as assigned by the agency
