Job Description:
The Case Manager position provides direct support and assistance in case management to residents participating in the program(s) and maintenance of daily records. The Case Manager works closely with the Program Director to ensure all job duties are performed according to program policies and procedures.
This role requires a female candidate who speaks Spanish, due to the population being served.
Key Responsibilities:
- Provide case management services to residents, including intake, assessment, and treatment planning
- Coordinate transportation for residents as needed
- Maintain accurate and up-to-date records of resident progress
- Collaborate with DCF, school personnel, and other provider agencies to ensure continuity of care
- Participate in resident team meetings and provide updates on resident progress
- Assist residents with daily living skills, such as budgeting and housing
- Conduct weekly individual meetings with residents to discuss treatment goals and progress
- Complete referrals for residents, including Early Intervention, Healthy Families, and mental health services
- Participate in training sessions to develop skills and knowledge in areas such as trauma-informed care and collaborative problem-solving
Requirements:
- Bachelor's degree in psychology or related field
- At least two years of experience working in a residential setting
- Ability to work flexible hours to meet the needs of clients' schedules
- Pass initial competency evaluation and continued assessments of professional development and future trainings
Preferred Qualifications:
- CPR/First Aid certification
- Dynamics of a Crisis including Physical Restraint Training
- Medication Administration Program (MAP)
- HIPAA, OSHA, Ethics and Boundaries
- Trauma Informed Care/Collaborative Problem Solving
