SUMMARY:
The Licensed Clinical Social Worker assists patients, and their families understand a particular illness, work through the emotions of a diagnosis, and provide therapeutic consultation in order to improve psychosocial functioning within a primary care setting. Works in collaboration with CPH clinical staff (doctors, nurses, and allied health professionals).
ESSENTIAL FUNCTIONS:
- Coordinates all psychosocial services under the general direction of the Clinical Psychologist/Psychiatrist. Establishes and maintains positive, effective communication and interpersonal relationships with patients, associates, managers, and physicians to facilitate continuity of client’s medical and psychosocial plan of care. Resolves patient concerns and complaints at the lowest level.
- Participates in staff meetings and team huddles to maintain associates are up to date with general changes.
- Responsible for assessing problems and determining appropriate types and methods of treatment and implementing intervention methodologies.
- Assist in the assessment, planning, arranging, and implementing of psychosocial treatment plans.
- Counsel patients and interpret professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders.
- Coordinate with clinical, health education, community health and other staff members in the provision of services to patients
- Provide referrals for services needed.
- Coordinate with other organizational entities involved in psychosocial and mental health services.
- Provide in-service presentations to staff as needed or required.
- Perform other related duties as assigned.
Qualifications
MINIMUM QUALIFICATIONS: (EXPERIENCE/SKILLS/ABILITIES/EDUCATION/CERTIFICATIONS):
- At least two (2) years of post-master’s supervised experience under the supervision of a licensed provider.
- Current and valid Florida Licensed Clinical Social Worker Licensure
- Proficient in Electronic Medical Records (EMR).
- Excellent verbal and written communication skills.
- Excellent collaboration skills required for community relationship building.
- Automated office and PC experience required.
- Current Basic Life Support (BLS)/CPR certification from the American Heart Association or the American Red Cross.
PREFERRED QUALIFICATIONS: (EXPERIENCE/SKILLS/ABILITIES/EDUCATION/CERTIFICATIONS):
- Proficient in the operations of Federally Qualified Health Centers (FQHC)
