Centerpoint Health is seeking a Licensed Practical Nurse (LPN) to provide both clinical and administrative support to our providers and to facilitate the delivery of health care services and patient care management in our health centers located in Norwood, Franklin, and Middletown OH.
Description of Position/Purpose:
The Licensed Practical Nurse works under the direction of a physician, AP or RN, the LPN participates in direct and indirect nursing care, health maintenance, teaching, counseling, and collaborative planning.
Job Duties:
Obtain an individual's basic health status and related data; record data in the electronic medical record {EMR).
Participate in analyzing and interpreting patient data; make informed judgements and incorporate the prescribed medical regimen into the nursing plan of care.
Participate in the health teaching required by the individual and family to maintain an optimal level of health care.
Communicate outcomes of basic nursing intervention and propose change in plans of care after consulting with physician, AP, RN.
Collaborate, cooperate and communicate with other health care providers to ensure quality and continuity of care.
Follow health center medical and clinical policies and protocol.
Oversee schedules
Document staff successes/concerns and patient concerns and function as the front line of conflict resolution with clinical staff.
Work collaboratively with the Ambulatory Supervisor {and/or Human Resources in their absence) to address accountability concerns.
Maximize provider visits so productivity targets are met.
Close the visit with the patient. Perform patient education within license restrictions about illness and medical condition in relation to diet, exercise, use of medications, diagnostics, or in-home testing to ensure good patient outcome. Reviews the after-visit summary {AVS).
Carry out provider's orders, including authorization of refills according to established protocols and administering treatments.
Correctly complete requisitions, sending specimens to the correct lab, per insurance. Perform accurate lab linkage, complete medical necessity advance beneficiary notices (ABNs). Follow insurance guidelines for preauthorization, diagnostic scheduling, referrals, and specialty visits.
Be aware of health insurance guidelines for pre-authorizations, referral processes; have ability to schedule diagnostics/specialist visits within network guidelines as ordered by the provider.
Other duties as assigned.

Certifications/Credentials:
Education: Licensed Practical Nurse
Certification and Licensure: Must maintain unrestricted LPN licensure in Ohio.
Experience: Prefer 3 years as LPN providing direct patient care in health care center. Primary Care experience preferred. Proven customer service experience required.
Additional Skills/Qualifications:
Patient-centered medical home (PCMH) experience and/or community health center experience.
Attention to detail and ability to follow directions and think independently as appropriate.
Interpersonal skills required to work collaboratively with clinical staff of all levels.
Able to assist in optimizing delivery of patient services, including knowledge of scope of practice of clinical team.
