Job Description:
Centers Dialysis Care is seeking a skilled Registered Dialysis Nurse to join our interdisciplinary team in a skilled nursing facility located in Bronx, NY. Our dialysis program provides high-quality care to patients in need of hemodialysis treatment.
Duties and Responsibilities:
- Collaborate with the healthcare team to assess patient status and implement individualized care plans.
- Perform care coordination and patient education to ensure optimal outcomes.
- Administer medications and other treatments as directed by provider/physician staff.
- Maintain accurate records and document patient responses to treatment requirements.
Requirements:
- Active NY RN License
- Dialysis experience required
- Nx Stage machine a plus
- CPR Certification
- Clinical nursing experience in a dialysis setting is preferred
