Centers for Behavioral Health, LLC in Cockeysville & Baltimore, MD is seeking a Rehab Coordinator / Case Manager to work in our supported housing program and our Life Skills Center, supporting clients with mental health diagnoses. This role will spend 50% of the time at the supported housing program in Cockeysville, MD and 50% of the time at the Life Skills program in Baltimore. Candidates must have upbeat, positive demeanors, display patience and excellent problem solving skills. Previous experience working with the chronically mentally ill will be given preference. High School diploma required / Bachelors preferred Valid driver's license with clean driving record Reliable transportation CPR Certified (active) Job Responsibilities : This position includes, but is not limited to, the following responsibilities. Meeting with clients one on one and managing a case load. Maintain a positive and professional manner in all methods of communication (persons served and co-workers) Exhibit strong time management skills Logging services in Credible Crisis Intervention Medication monitoring Preparing simple meals for residents (lunch and dinner) Transportation to dr. appts on occasion Coaching and guiding residents with Life Skills, cleaning, and hygiene Advocacy and Community Integration Encouraging activities Participating in group activities onsite (special occasion lunches or events) Requirements This position requires a minimum of a high school diploma with relevant behavioral health experience in a supported housing environment. A degree in a behavioral health related field is preferred but not required. All personnel are required to be CPR certified. Must have a current driver's license and active automobile insurance coverage. Reliable transportation.
