MDS Coordinator Responsibilities:
- Coordinate and complete MDS assessments in accordance with state and federal regulations
- Collaborate with interdisciplinary teams to develop individualized care plans
- Ensure timely and accurate documentation for reimbursement and compliance
- Monitor resident status and update assessments as needed
- Provide education and support to staff on MDS-related processes

Qualifications:
- Licensed Registered Nurse (RN) in Ohio
- Experience with MDS 3.0 and care planning in a long-term care setting
- Knowledge of Medicare, Medicaid, and reimbursement guidelines
