Job Description
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As a Home Health LPN, you will work closely with our interdisciplinary care teams to deliver high-quality patient care and achieve exceptional patient outcomes.
Key Responsibilities
- Provide professional nursing services under the direction of a RN in compliance with state regulations and company policies
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement individualized plans of care that address patients' goals and priorities
- Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions
- Maintain accurate and detailed records of patient interactions and progress
Requirements
- Graduate of an accredited LPN program
- Current nursing license in good standing
- Valid driver's license and reliable transportation
- CPR certification
- Two years of experience as an LPN in a clinical setting, preferably in home health or hospice
Salary : $31 - $44
