Essential Responsibilities
- Assess patients' emotional and social needs, developing personalized plans of care to address these needs
- Work collaboratively with the care team to ensure that patients receive the highest quality care
- Identify and connect patients with community resources to support their recovery and well-being
- Provide education and support to patients and families on managing chronic conditions and navigating the healthcare system
- Collaborate with physicians and other healthcare professionals to ensure that patients receive coordinated and comprehensive care
Requirements
- Master's or doctoral degree in social work from an accredited program
- Licensure as a social worker in the state of practice (if required by state law or regulation)
- Experience working in a healthcare setting, preferably in home health or hospice
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
- Ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records
- Valid driver's license and reliable transportation (for visiting patients in their homes)
