At CenterWell Home Health, we are dedicated to delivering high-quality patient care in the comfort of our patients' homes. As a Home Health Speech Language Pathologist, you will play a vital role in evaluating and treating patients with speech, language, and swallowing disorders.
Care Coordination
You will participate in the development and periodic review of the Plan of Treatment and Plan of Care, working closely with our interdisciplinary team to ensure comprehensive care for each patient. Your skills in assessing and treating disorders of speech, voice, language, hearing, and swallowing will be invaluable in preventing, identifying, evaluating, and minimizing the effects of such conditions.
In addition to your clinical expertise, you will provide instruction and training to patients in the use of alternative communication systems when appropriate. You will also maintain accurate and timely documentation of patient progress, ensuring seamless communication with our care teams and other healthcare providers.
- Evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities.
- Participate in care coordination activities, including discharge planning and follow-up care.
